§ 157.049 CONTENTS OF APPLICATION FOR CONDITIONAL USE PERMIT.
   An application for conditional use permit shall be filed with the Zoning Inspector who shall transmit it to the Board of Zoning Appeals, by at least one owner of property for which the conditional use is proposed. At a minimum, the application shall contain the following information:
   (A)   Name, address, and phone number of applicant;
   (B)   Legal description of property and/or address;
   (C)   Description of existing use;
   (D)   Zoning district;
   (E)   Description of proposed conditional use;
   (F)   A plan of the proposed site for the conditional use showing the location of all buildings, parking and loading areas, traffic access and traffic circulation, open spaces, landscaping, refuse and service areas, utilities, signs, yards, and such other information as the Board may require;
   (G)   A statement explaining how the specific conditions for the specific use as required in § 157.130 will be met;
   (H)   A fee as established by Council; and
   (I)   A list containing the name and mailing address of all owners of property within 200 feet of the property in question.
