Written application for such license shall be made to the business office manager stating the name of the applicant, the place of business, number of persons to be employed and the kind of business to be carried on and all such other information as may be requested by the application form as amended from time to time. All licenses shall issue by authority of the president and board of trustees of the village or their designee. Provided, however, that no business license shall issue to any person engaged in or intending to engage in the operation of a food service establishment and/or retail food store within the village until and unless evidence of a valid permit issued by the DuPage County health department pursuant to the provisions of this code, as may be from time to time hereafter amended, regulating and supervising food service and retail food store sanitation, is presented with said application to the business office manager. Further, it shall be the responsibility of any licensee engaged in or intending to engage in the operation of a food service establishment and/or retail food store within the village to maintain a valid permit issued by the DuPage County health department, during the entire term of the village license and issuance of a village license shall not relieve the licensee of such obligation. The failure to maintain a valid permit issued by the DuPage County health department, as required, shall be grounds for revocation of a village license. (Ord. 2002-3425, 11-4-2002)
