(a) The Wage and Salary Review Committee is authorized to offer health care benefits to certain part-time employees that meet the requirements stated by the insurance provider.
(b) These benefits would be paid by employee deduction, with the Village share being equal to fifty percent (50%) of the single rate plan as suggested by the rate structure attached.
(c) At any time that the employee fails to meet the requirements of the insurance provider as to time worked during the premium period, that employee be required to reimburse the Village for expenditures made on their behalf.
(d) If the employee fails to meet the requirements of the insurance provider for two consecutive premium periods, that they be removed from the Village’s health insurance coverage until the next open enrollment period providing they meet all other requirements at that time.
