147.04 HEALTH INSURANCE.
   (a)   For employees hired prior to July 1, 1986, each full-time employee covered by the terms of this section shall be provided, upon completing ninety days of full-time employment in a satisfactory manner, a family hospitalization plan with either Blue Cross/Medical Mutual or Kaiser Community Health Foundation Plan (Group #188-C Plan ($3.00 prescription)), at no cost to the employees.
   (b)   For employees hired after June 30, 1986, the following hospitalization plan with either Blue Cross/Medical Mutual or Kaiser Community Health Foundation Plan (Group #188-C ($3.00 prescription)), shall be provided to employees covered by the terms of this section on the following terms:
      (1)   Upon completing in a satisfactory manner, ninety days of full-time employment, the Village shall contribute fifty percent (50%) of the cost of the monthly premium for any such plan provided, and the employee shall contribute the remaining fifty percent (50%).
      (2)   Upon completing, in a satisfactory manner, an additional one year of full- time employment, the Village shall contribute seventy-five percent (75%) of the cost of the monthly premium for any such plan provided, and the employee shall contribute the remaining twenty-five percent (25%).

      (3)   After two years of having and continued full-time employment status with the Village of Linndale, such full-time employee’s share of health benefit payments are discontinued.

   (c)   The Mayor is hereby authorized to offer health coverage to appointed positions within the Village following the same criteria as set for part time employee subscribers.
      (1)   These positions are limited to Law Director and Assistant Law Director; Prosecutor and Assistant Prosecutor, and Clerk-Treasurer.
      (2)   The offer of health coverage is contingent upon the acceptance of the individual by the current health provider as a regular subscriber and the individual is responsible for assuring that the health provider’s criteria are met. (Ord. 2006-16. Passed 5-9-06.)
