Any person desiring to secure a license to maintain or operate a junkyard, as required by Section 721.01 hereof, shall make a written application therefor to the Mayor. A fee of one hundred dollars ($100.00) shall accompany the written application to cover the cost of investigating the application thereof upon issuance of such license. Applications shall set forth the following:
(a) The full name, age and residence of the applicant. If the applicant is a partnership, the full name, age and residence of each partner, and if the applicant is a corporation, its principal address and the full names of its principal officers.
(b) The name under which applicant proposes to do business.
(c) That the applicant is capable of and will carry adequate liability insurance as hereinafter provided.
(d) Such other information as the Mayor shall require.
