Every person doing business as a budget service company shall make application to the Mayor for a license to engage in such business. Such application shall be in a letter and shall contain the following information together with such further information as the Mayor may require:
(a) The name under which such business is to be conducted;
(b) The name and address of owner, partner, or associate, or in the case of a corporation, the names and addresses of all officers, statutory agent, directors, and controlling stockholders of the company;
(c) A sworn statement that no owner, partner, associate, or officer has been convicted of a felony; and
(d) The address of each office of the business.
