Every person licensed under this subchapter shall make out and deliver to the Village Police Chief on the first day of each month a legible and correct report of every sale or gift made under authority of the license to sell the kind of weapon or other articles named in this subchapter during the preceding month, which report shall contain the date of the sale or gift, the name of the purchaser or donee, his address and age, the number, kind, description and price of the weapon or other article, the number of the purchaser's permit and the purpose of the purchase of the weapon or other article.
