Applicants for registration under this chapter shall file a written sworn application on forms furnished by the Building Department. Such application shall be signed by the applicant, if an individual; by all partners, if a partnership; by all associates, if an association; and by the president, if a corporation. Such application shall be filed with the Department and shall provide the following information:
(a) The full name and permanent address of the applicant, both business and residence;
(b) The period of time, if any, during which the applicant has been engaged in the business;
(c) A description of the type of business entity, i.e. corporation, partnership or sole proprietorship;
(d) Names, home addresses and titles of all corporation officers, partners or sole owners;
(e) The applicant's State Department of Licensing and Regulation license, from which a copy may be made for the Builders Registration Commission's records. If the applicant has applied to the State for his or her license but has not yet received the same, the applicant may obtain a certificate under this chapter on the condition that the license be obtained within 120 days.
Such other information as to the identity or character of the applicant, or of any of the officers or members of any such applicant, as the Building Inspector may deem proper to fulfill the purposes of this chapter in the protection of the public good.
