A written application for a license required by this chapter shall be filed with the City Clerk and shall state the following:
(a) The name and address of the applicant;
(b) The location at which the outdoor sale is to be conducted;
(c) The commencement date and termination date of such sale; and
(d) A description of the items proposed to be sold outdoors.
