An application for a license required by this chapter shall be made in writing, shall be upon printed blanks to be furnished by the City Clerk and shall set forth, among other things, the place where such business is to be conducted, the name of the person by whom such business is to be carried on and the place of residence of such applicant. Such application shall also provide for the consent of the applicant to have members of the Police Department and the Fire Department, together with the Health Officer, enter and inspect any part of such premises and to take therefrom samples of foods or victuals, or both, for analysis.
