The applicant for a license under this chapter shall file with the Health Officer a written application, under oath, on a form prescribed and supplied by the Health Officer, and shall submit satisfactory proof of the required age, educational qualifications and good moral character, as provided in this chapter. If such applicant is qualified, he or she shall receive a certificate of approval from the Health Officer, a copy of which certificate shall be delivered to the license collector with the license application and the license fee.
