812.03 PERMIT APPLICATIONS.
   A written application for a permit to solicit for any cause whatever, as provided in Section 812.01, shall be sworn to and filed with the City Clerk fifteen days prior to the time when a permit is desired. Such application shall be made on a form furnished for such purpose by the City Clerk. The application shall contain, in addition to such other information as may be requested by the Mayor and Council, the following information:
   (a)   The name of the person or organization applying for a permit to solicit and the address of his, her or its headquarters;
   (b)   The names and addresses of his, her or its principal officers and management;
   (c)   The purpose for which such solicitation is to be made and the use or disposition to be made of any receipts therefrom;
   (d)   The name of the person by whom the receipts of such solicitation shall be disbursed and, if there are any participating organizations, the names of officers and employees disbursing such receipts for such participating organizations;
   (e)   The name and address of the person who will be in direct charge of conducting the solicitation;
   An outline of the method to be used in conducting the solicitation;
   (g)   The time when such solicitation shall be made, giving the proposed dates for the beginning and ending of such solicitation;
   (h)   The amount of any wages, fees, commissions, expenses or emoluments to be expended or paid to anyone in connection with such solicitation, together with the manner in which such wages, fees, expenses, commissions or emoluments will be expended, to whom the same are paid and the amount or percent thereof; and the salaries, emoluments or expenses to be paid to anyone in connection with the administration of the receipts of such solicitation, including all such salaries, emoluments or expenses in regard to any organization participating in the receipts from such solicitation;
   (i)   A financial statement for the last preceding fiscal year, or any lesser period if not existing that long, of any funds collected for the purposes set out in Section 812.01 by the organization or person seeking a permit for such solicitation. Such statement shall give the amount of money so raised, together with the cost of raising it, and the final distribution thereof, and shall include an itemized statement of the expenses of administering such fund for the preceding fiscal year showing all expenses incurred by organizations participating in the receipts from the solicitation.
   (j)   The total amount of funds proposed to be raised and a full statement of the character and extent of the charitable, educational or philanthropic work being done by the applicant organization in the City, which statement shall include full information concerning such work done by all organizations from such solicitation; and
   (k)   Such other information as may be required by the Mayor and Council in order for the Mayor and Council to fully determine the kind, character and worthiness of the proposed solicitation and as to whether or not such solicitation is in the interest of protecting the safety, health, life and property of the residents of the City, and in the interest of preserving and enforcing good government, and for the security of the City and its inhabitants.
