§ 110.21   APPLICATION FOR LICENSE.
   (A)   All applications for licenses, unless otherwise specifically provided, shall be made to the Village Manager in writing on forms to be furnished by him or her, and shall contain:
      (1)   The name of the applicant, and of each officer, partner, or business associate;
      (2)   The place of business and the present occupation;
      (3)   The place of residence for five years preceding the date of application;
      (4)   The nature and possessory interest in the location of the intended business or enterprise;
      (5)   The official stamp of the Tax Commissioner attesting to the fact that the applicant has registered such business with the tax provisions of Chapter 39; and
      (6)   If for a vendor, a description of the goods or merchandise to be sold.
   (B)   The renewal of an annual license may be granted to a licensee in good standing on the original application, unless otherwise provided. With each original or renewal application the applicant shall deposit a fee of $15 for the license requested. It shall be unlawful to knowingly make any false statement or representation in the application.
