(A) Whenever a department head investigates, receives information of an investigation, or causes an investigation to be conducted into the actions or conduct of a village employee under his or her supervision, that are related to the performance of the employee’s duties as a village employee, the department head shall so notify the Village Manager immediately and make a written report in 24 hours.
(B) Failure to report such an investigation shall constitute insubordination.
