An application for a permit shall be submitted to the Building Commissioner and shall disclose the name and address of the applicant, the location of the proposed courtesy bench or benches and any other information required by the Commissioner. One application may be made for one or more benches and one permit may be issued for one or more benches; however, a separate fee shall be paid as stipulated in Section 812.05. An individual or legal entity may make application for no more than twenty benches per year.
