(a) Names on Ballots.
(1) The names of all candidates nominated for elective office, except those who have withdrawn, died or become ineligible, shall be placed on the official ballots in accordance with general statutory law, and such names shall appear with local party designation or symbol under a heading reading: "Nominees for DESIGNATE OFFICE".
(2) If two or more candidates for the same office have the same surname or surnames so similar as to be likely to cause confusion, their residence addresses shall be placed with their names on the ballot.
(3) The position of names on the ballots shall be in accordance with the requirements of general statutory law.
