§ 709.02 REGISTRATION REQUIREMENTS.
   (A)   Registration. All natural persons desiring to engage in soliciting at the residences in the city or within the police jurisdiction thereof shall apply to the Clerk or the Clerk's designee, at a cost of $25, for an annual certificate of registration. The certificate, executed by the Clerk or his or her designee, shall be maintained by the Clerk, and a legible copy shall be provided to and carried by the registrant at all times while soliciting. Upon request for inspection by an occupant of a residence or a police officer while the registered solicitor is transacting his or her business or seeking to transact his or her business at any residence, the registrant shall show his or her registration certificate.
   (B)   Application for certificate. The city shall provide a standard form for use for registration of solicitors. The applicant for the certificate shall state upon his or her oath or affirmation that the information is truthfully provided to the best of his or her knowledge and belief. The applicant shall supply upon the form the following information:
      (1)   Applicant's true, correct, and legal name, including the name and address of any entity by which applicant may be employed in sales or for whose benefit applicant will be soliciting donations;
      (2)   Address of applicant's permanent home and address of any temporary local residence;
      (3)   The purpose for which soliciting will be done;
      (4)   A brief description of the method of presentation that will be made (such as presentation of samples and taking of written orders);
      (5)   The dates for when soliciting is planned;
      (6)   Whether a permit, license, or certificate of registration in connection with soliciting has ever been revoked, explaining the circumstances.
   (C)   Non-endorsement. In addition, the applicant shall certify on the application form that if his or her registration is granted, he or she will not state, imply, or represent to any person that the issuance of the certificate is an endorsement by the city nor any of its agencies or departments of the product, service or solicitor.
   (D)   Proof of identification required.
      (1)   The application shall be accompanied by a copy of one of the following:
         (a)   A valid driver's license of any state; or
         (b)   A valid United States Uniform Service Identification; or
         (c)   A valid passport; or
         (d)   A valid identification issued by any agency of a state for the purpose of identification of the holder.
      (2)   Likewise, the applicant shall either submit a completed FBI standard fingerprint card of his or her complete fingerprints or submit to fingerprinting by the Police Department for a fee of $5.
   (E)   Registration complete with issuance. Registration shall be complete when the Clerk issues the certificate of registration, which shall not be unreasonably withheld or delayed more than three days.
   (F)   Form of certificate. Each certificate shall be numbered, shall list the name of the individual solicitor and of any organization on whose behalf he or she is authorized to solicit, the date on which the certificate expires, and shall be dated and signed by the Clerk or the Clerk's designee.
   (G)   Maintenance of registry. The Clerk shall maintain in his or her office, available for public inspection, a record of every application received and acted upon, together with any information pertaining thereto, all certificates of registration issued, and all denials of applications. Each application shall be numbered in consecutive order as filed, and each certificate issued shall be assigned a number exclusive to the registrant. Every certificate renewed shall be identified with the duplicate number of the application upon which it was initially issued. The Clerk shall furnish to the Chief of Police a listing of all certificates issued and renewed and the Chief of Police shall maintain such listing in his or her office for public inspection and for identification.
   (H)   Renewal. Certificates shall be valid for the calendar year in which they were issued and shall expire at midnight on December 31st of such year. Any certificate in good standing may be renewed upon request of the registrant and upon payment of $10. The applicant shall be required to fill out a renewal application providing the information required in the initial registration. Copies of the renewed certificate shall be provided at a cost of $.50 each.
   (I)   Non-transferability. Certificates of registration shall be issued only in the name of the applicant and the firm, corporation or association on whose behalf he or she is authorized to solicit. Certificates shall be non-transferable from individual to individual, from individual to firm, from firm to firm, and from firm to individual. It shall be a violation for a solicitor whose certificate authorizes soliciting on behalf of a firm named in the certificate to solicit on behalf of any other firm, organization, or association or for any purpose other than that specified on the application.
