§ 72.09  INFORMATION TO BE RECORDED UPON ARREST.
   Except as otherwise provided in this chapter, any police officer, when making an arrest hereunder, shall take the name, address and operator’s license number of the alleged violator and the registered number of the motor vehicle involved, or if the vehicle is unattended, he or she shall take its registration number and any other information which may identify its user, and shall issue to him or her, in writing, a notice to answer the charge against him or her at a time and place at least five days after the arrest. The second copy of the notice shall be forwarded to the City Court and a record thereof retained by the Chief of Police. Upon failure of the person to appear within the time stated in the notice, he or she shall be guilty of a misdemeanor and a warrant, upon proper complaint, shall be issued for his or her arrest.
