§ 5-2-5-3. APPLICATION PROCEDURES.
   (A)   Applications shall be submitted in accordance with procedures established by the Board and revised from time to time.
   (B)   An application fee of $50 shall be submitted to cover the administrative cost of processing the application. The applicant shall also pay a plan review fee as provided by § 5-2-5-16.
   (C)   Applications for construction permits shall be submitted at least 60 days in advance of the proposed start of construction; provided, however, that a shorter time period may be approved by the Board.
   (D)   Applications shall include a certificate of sufficiency of plan filed by a professional engineer or, for internal subdivision sites, a certificate of sufficiency of plan filed by a land surveyor registered in accordance with I.C. 25-31-1.
   (E)   The Board may, as a prerequisite to the issuance of a construction permit, require developers, wherever applicable, to send written notification to property owners whose properties abut the route of the proposed sewer. The notice shall not be given when Indiana Department of Environmental Management provides the notice.
   (F)   Applications may include any additional information deemed necessary by the Board to carry out the provisions of this chapter.
