(A) Applications shall be submitted in accordance with procedures established by the Department and revised from time to time. Design plans and specifications for the construction of sanitary sewers shall be developed by or under the direction of a professional engineer registered in accordance with I.C. 25-31-1 and shall have a title sheet which includes the professional engineer’s seal and signature.
(B) An application fee of $50 shall be submitted to cover the administrative cost of processing the application. The applicant shall also pay a plan review fee as provided for in § 5-1-6-16.
(C) Applications for construction permits shall be submitted at least 60 days in advance of the proposed start of construction; provided, however, that a shorter time period may be approved by the Director.
(D) Applications shall include a certificate of sufficiency of plan filed by a professional engineer registered in accordance with I.C. 25-31-1.
(E) The Director may, as a prerequisite to the issuance of a construction permit, require developers, wherever applicable, to send written notification to property owners whose properties abut the route of the proposed sewer.
(F) Applications shall include any additional information deemed necessary by the Director to carry out the provisions of this chapter.
