(A) All applications for a license required by § 3-3-8-8 shall be made on forms designated by the Department of Public Works which shall include the following information:
(1) The full name and address of the alarm business;
(2) The full name, business address and home address of the manager; and
(3) A telephone number by which the Lawrence Police Department can notify personnel of the alarm business of a need for assistance at any time.
(B) An alarm business shall promptly notify the Lawrence Building Commissioner’s office in writing of any change in the information contained in the application form.
