(A) If during the inspection of any tattoo or body piercing establishment or facility the Health Department discovers any violation of any provision of this chapter or of 410 IAC 1-5 (or as amended hereafter), the Health Department shall issue a written report listing such violations.
(B) A copy of the written report shall be filed in the records of the Health Department after appropriate review by supervisory personnel.
