§ 113.037 LICENSE APPLICATION PROCEDURE.
   (A)   General. Every application for a license to sell 3.2% malt liquor shall be made on a form supplied by the city and shall state the name of the applicant, his or her age, representations as to his or her character with such references as may be required, his or her citizenship, whether the application is for on sale or off sale, the business in connection with which the proposed license will operate and its location, whether the applicant is the owner and operator of the business, how long he or she has been in that business at that place, the amount and nature of any dancing, singing, vaudeville performance or other entertainment to be provided for guests, and such other information as the Council may require from time to time. Except for resorts, a scale drawing of the floor plan of the proposed licensed premises shall be attached to each application. It shall be unlawful to make any false statement in the application. Applications shall be filed with the City Clerk-Administrator.
   (B)   Corporations. In addition to the above, an application for an on sale or off sale license on behalf of a corporation shall include complete information as to the names, addresses and occupations of the stockholders and officers of the corporation and other information required by the Council. Corporate officers shall notify the Council whenever any change is made in the officers of any such corporation and whenever there is a proposed sale of transfer of corporate stock.
