(A) Application for license shall be required upon initial application, upon renewed application and upon any change of ownership of business control of a licensed premise.
(1) Every application for a license to sell liquor shall be verified and filed with the City Clerk-Administrator and shall state:
(a) The name of the applicant;
(b) His or her age;
(c) Representations as to his or her character, with such references as may be required;
(d) His or her citizenship;
(e) Whether the application is for on sale or off sale;
(f) The business in connection with which the proposed license will operate and its location, the nature of the building and its location on the property;
(g) Whether applicant is the owner and operator of the business, how long he or she has been in that business at that place;
(h) The amount and nature of any dancing, singing, vaudeville performance or other entertainment to be provided for guests; and
(I) Such other information as the Council may require from time to time.
(2) In addition to containing such information, each application for a license shall be in the form prescribed by the Department of Public Safety. No person shall make a false statement in an application.
(B) On sale wine licenses shall be issued only to restaurants meeting the qualifications of M.S. § 340A.404, subd. 5, as it may be amended from time to time, and shall permit only the sale of wine not exceeding 14% alcohol by volume for consumption on the licensed premises only, in conjunction with the sale of food. In addition to the wine license, a holder of an on sale wine license, issued pursuant to this subchapter who is also licensed to sell 3.2% malt liquors at on sale pursuant to M.S. § 340A.411, as it may be amended from time to time, and whose gross receipts are at least 60% attributable to the sale of food, shall also have the permission to sell intoxicating malt liquors at on sale without an additional license.
(C) Each application for a license shall be accompanied by a liability insurance policy, or in lieu thereof, cash or United States Government bonds of equivalent market value as provided by the state statutes.
