§ 110.218  APPLICATIONS.
   Applications for a landscaper license shall be made to the City Manager on forms provided by the city and shall contain the following information:
   (A)   Name and address of the applicant; if the applicant is a partnership, the application shall show for each partner his or her name and address and interest. If the applicant is a corporation or other business entity, the application shall show the name and address of the registered agent, and each manager or director; and with respect to each shareholder or person owning at least a 5% interest in the entity or any class of its corporate shares, the person’s name and address;
   (B)   The number, kind and license plate number of vehicles owned and controlled by the applicant’s landscaping business;
   (C)   The location of the applicant’s principal business office and garage;
   (D)   The color scheme, insignia, trade name and telephone number located upon and used to designate the vehicles of the applicant;
   (E)   A statement that the applicant has received copies of, reviewed and understands the applicable ordinances of the city and will notify the applicant’s employees thereof;
   (F)   A statement that the applicant has received copies of, reviewed and understands the guide to ravines and bluffs;
   (G)   Delivery of proof of insurance with the coverage types and amounts as established from time-to-time by the City Manager, which insurance shall name the city as an additional insured;
   (H)   Delivery of a surety bond or other security in the amount of $5,000 for the benefit of the city in the form prescribed from time to time by the City Manager; and
   (I)   Such other information as the City Manager may require from time to time.
