§ 110.082 LICENSE APPLICATION.

   Application for a music vending machine license shall provide the following information:

   (A)   Full name and address of the applicant;

   (B)   Name of music vending machine owner;

   (C)   Where the machine is to be used;

   (D)   Number of machines to be installed; and

   (E)   Description of machine, manufacturer’s name, the make and factory number.
