§ 99.067 APPLICATIONS.
   (A)   All applications for development shall include the following information in a form acceptable to the city and as further detailed in the permit application materials available from the city.
   (B)   Any of the requirements may be waived by the Director of Community Development if the existing trees and vegetation on the site are not subject to the provisions of the city’s tree preservation ordinance due to size, species or condition.
      (1)   Site plan of proposed improvements.
         (a)   A plan shall be submitted showing the property lines, zoning setbacks, utility and access easements, conservation easements, utility stubs, preservation and no disturbance areas established by the plat of subdivision or a previously approved development plan, the location of all proposed buildings including any roof overhangs, all patios, terraces, tennis courts, swimming pools and other accessory structures.
         (b)   All existing trees eight-inch DBH or larger and all clusters of six or more trees of at least four-inch DBH shall be reflected on the plan.
      (2)   Grading plan.
         (a)   A plan shall be submitted showing the existing elevation of the site and the location and amount of all proposed grading and fill.
         (b)   All existing trees eight-inch DBH or larger and all clusters of six or more trees of at least four-inch DBH shall be reflected on the plan.
      (3)   Location of utilities.
         (a)   A plan shall be submitted showing the proposed location of all utilities.
         (b)   All existing trees eight-inch DBH or larger and all clusters of six or more trees of at least four-inch DBH shall be reflected on the plan.
         (c)   Utilities shall be located in the driveway areas to minimize the impact on the trees on the site.
         (d)   Alternative locations for utilities may be approved by the Director of Community Development if found to be consistent with the purpose of this chapter.
      (4)   Tree survey.
         (a)   The survey shall include information on all existing trees eight-inch DBH or larger and clusters of six or more trees at least four-inch DBH.
         (b)   Information shall include the location, size, species and condition of each tree.
         (c)   Each tree shown on the survey shall be tagged on the site.
