(A) Approval categories. A site plan for the construction of the following onsite wastewater treatment systems, and the following development projects or activities require Health Officer approval for determination of adequate treatment and disposal of wastewater and to protect the health, safety, and general welfare of the people of Lake County.
(1) New Onsite Wastewater Treatment Systems.
(2) Replacement Onsite Wastewater Treatment Systems.
(3) Alterations or Additions to Properties.
(a) Alterations or Additions Not Requiring Replacement of System Components.
(b) Alterations or Additions Requiring Replacement of System Components (Alteration- Replacement Onsite Wastewater Treatment Systems).
(4) Repairs to an Onsite Wastewater Treatment Systems.
(5) Subdivisions.
(6) Onsite Wastewater Treatment Systems for Atypical Wastewater Flow.
(7) Non-Soil Based Onsite Wastewater Treatment Systems.
(8) Onsite Wastewater Treatment System Pre-Treatment Component Elimination.
(9) Artificial Site Drainage Effectiveness Investigation Plans.
(B) Required component maintenance. When a site plan is approved in accordance with the requirements of §§ 171.065 through 171.074 that includes the relocation or replacement of any existing onsite wastewater treatment system component, or the installation of a new onsite wastewater treatment system component, the owner of record of the onsite wastewater s treatment system shall be required to meet the requirements of §§ 171.220 through 171.224 and 171.255 through 171.261 for the maintenance and inspection of any new or existing component that is subject to the requirements of those sections.
