7-2-3: INFORMATION TO ACCOMPANY APPLICATION:
   A.   Material Required: Whenever in this Title a number of originals or copies of any documents to be submitted to the Village is stated, the stated number shall be and constitute the minimum number of copies required, but the Building Commissioner shall be and is hereby authorized, at his discretion, to determine in each case the required number of original(s) and/or copies of each document that shall be required to be submitted to the Village. Descriptive material, in drawn and written form, which in the aggregate sets forth the location and design of buildings, structures, water supply system(s), the sewage disposal system, plumbing and electrical installations and tests as required by the Building Commissioner to assure compliance with this Village Code and other applicable laws, rules and regulations and shall include but not be limited to the following, except as waived by the Building Commissioner:
      1.   List of Contractors and Subcontractors: Completed building permit application for submission to the Village shall include the list of contractors and subcontractors who will be working on the project, license numbers where applicable, together with their respective contact names and such other information as mailing addresses, e-mail addresses, and telephone numbers.
      2.   Soil Reports: Approved Lake County health department permit to include: Not less than two (2) sets of soil reports.
      3.   Watershed Development Ordinance Permit Application: Completed "watershed development ordinance permit" application.
      4.   Acknowledgment Of Covenants: Signed and dated "acknowledgment of possible covenants, conditions and restrictions of record" as specified in subsection C of this section.
      5.   Approval Letter: If required by an applicable annexation agreement or PUD ordinance, a "subdivision architectural control approval letter" for the project.
      6.   Building Plans: Not less than four (4) sets of the building plans for a proposed project stamped by an Illinois licensed architect, the number of which copies of plans shall be as determined by the Building Commissioner and shall be provided with the application. Said plans shall include the front elevation, the rear elevation, the side elevations, and all other exterior elevations of each building or structure on which work shall be done, and a site plan. Floor plans for all levels drawn on a scale of one-fourth inch equals one foot (1/4" = 1') shall be included, and each plan shall include the dimensions, functions, structural and electrical layout, wall sections and details of construction materials and include light and ventilation schedules.
      7.   Not less than three (3) copies of topographical site plans for the project, as determined by the Building Commissioner, shall be provided with the application. Such topographic site plans shall include but not be limited to at least one hundred feet (100') of the surrounding properties showing: top of foundation elevations, all existing and proposed building including underground facilities and the location of each, driveway culvert location, accurate dimensions, lot boundaries, and distances between all improvements; a tree survey as required by Chapter 1 of Title 11 of this Village Code ("Protection and Preservation of Existing Trees") which shows the size, quality, health, and species of trees which may be impacted by the project.
      8.   Plat of Survey: Not less than two (2) sealed originals of an individual lot survey with property stakes marking each corner of the lot prepared, signed, sealed, and certified by a professional land surveyor registered in the State of Illinois.
      9.   Fire Suppression and Alarm System: Not less than two (2) sets of drawings of the proposed fire suppression system and, if applicable, any alarm system, for fire department review and approval.
      10.   Site Plan: Not less than three (3) sealed originals of the site plan shall be submitted to the Village for review and approval, which site plan shall be drawn to scale by a registered professional engineer or licensed Illinois land surveyor, and which shall (the following requirements are separate and in addition to the requirements for an individual sewage disposal system permit):
         a.   Be prepared by and certified with the original seal and signature of a professional engineer or professional land surveyor registered in the state of Illinois, and shall bear a certification by said professional engineer or professional land surveyor that the plan complies the approved grading plan for the project as required by subparagraph (i) below. All information on the plan shall be certified.
         b.   Be titled "Permit Site Plan".
         c.   Be the size of a standard plan sheet of twenty four inches by thirty six inches (24" x 36").
         d.   Have a legal description and Permanent Index Number(s) of the subject property and street address shown thereon.
         e.   Have a scale of one inch equals twenty feet (1" = 20') (architectural scales are not acceptable).
         f.   All information shall be legible.
         g.   Have a north arrow indication to be either up or to the right.
         h.   Include a "location plan" or a "vicinity plan" for the subject property.
         i.   Include confirmation by the developer's engineer that the proposed design correlates with the approved subdivision grading plan. Compliance with that plan, by specific reference, shall be a part of the professional land surveyor's or professional engineer's certification.
         j.   Include additional information which may be required if a FEMA flood zone is within the limits of the survey in accordance with the elevation certificate instructions per the FEMA national flood insurance program.
         k.   Indicate all of the following locations:
            (1)   Property lines with bearings and dimensions (include arc length, radius, PC, PT, and included angle for curves).
            (2)   Lot corners (shall be monumented and indicate monument type).
            (3)   Building setback lines and building lines (located with dimensions from lot lines).
            (4)   All easements (purpose shall be identified).
            (5)   Trees with a diameter at breast height ("dbh"), i.e., 54 inches above the root ball, of four inches (4") or more, per Chapter 1 of Title 11 of the Village Code.
            (6)   Potable water well (with 75 foot radius).
            (7)   All components of the septic system.
            (8)   Driveways (including type of surface).
            (9)   Driveway culvert (including material which shall comply with the requirements of section 9-5-3 of this code, pipe bedding detail, size [minimum 12 inch], and length).
            (10)   Retaining walls (including upper and lower grades and material).
            (11)   Gas service, electrical service, private drains, outlet locations.
            (12)   Topographical features, including swales, shoulders and culverts within at least one hundred feet (100') of the property that may bear on the proposed development.
            (13)   Floodplain, floodway, and wetland delineation as applicable.
            (14)   Location of a temporary portable privy (i.e., port-o-potty) as specified in Section 7-2-5(G) of this Chapter.
            (15)   Location, number and size of portable waste container(s) as specified in Paragraph 7-2-5(G) of this Chapter.
         l.   Elevations shall:
            (1)   Include bench mark description and elevation.
            (2)   All elevations shall be to the North American Vertical Datum of 1988 (NAVD88), or as thereafter amended.
            (3)   Indicate the existing topography and elevation of the following:
               (A)   Floodway or floodplain, if applicable.
               (B)   Nearby high water/normal water elevation of detention facilities.
               (C)   Lot corners.
               (D)   Invert of all nearby roadway and driveway culverts.
               (E)   Roadway centerline, edge of pavement, and swale invert fronting property and within at least one hundred feet (100') of property.
               (F)   Topographical features and grades within at least one hundred feet (100') of the property that may bear on the proposed development.
               (G)   Spot grades and contours at one foot (1') intervals.
            (4)   Indicate the proposed elevation of the following with a line and letter quality differing significantly from "existing":
               (A)   Side and rear lot lines including high points, low points, berms, and swales.
               (B)   Top of building foundation, sill of lookouts, sill of walkouts, the lowest opening into foundation, and garage floor.
               (C)   All elements of the septic system.
               (D)   Area surrounding septic area to show adequate drainage.
               (E)   Grades adjacent to the foundation including specific location of lookouts, walkouts, and other openings.
               (F)   Driveway culvert inverts at both ends including grades above the culvert.
      11.   Septic System: Not less than two (2) copies of the septic system layout approved by the Lake County health department, Waukegan, Illinois ("application for permit for individual sewage disposal system"), and approved by the village enforcement officer in accordance with title 12 of this code relative to individual sewage disposal systems, with sump pump discharge routing approved by the building commissioner.
   B.   Waiver Of Requirement: The obligation to provide plans may, in the discretion of the building commissioner, be waived for minor alterations not involving structural changes and for residential accessory buildings if such work is sufficiently described in the application for a permit.
   C.   Acknowledgment: Each applicant for a building permit shall deliver a copy of the following acknowledgment executed by the applicant and the owners of record of the property which is the subject of the application:
      ACKNOWLEDGMENT OF POSSIBLE COVENANTS, CONDITIONS AND RESTRICTIONS OF RECORD:
      The undersigned, having applied to the Village of Lake Barrington for a building permit, acknowledges:
      1. That there may be private covenants, conditions and restrictions running with the title to the property which is the subject of our permit application which regulate, govern, control and/or affect what type of improvements may be made on the subject property;
      2. That the Village, by issuance of a building permit has no power to and does not abrogate, vary, terminate, waive or release any such covenants, conditions and restrictions of record which may be applicable to the subject property;
      3. That the undersigned remain obligated to comply with such covenants, conditions and restrictions of record, notwithstanding the fact that they have received a building permit from the Village of Lake Barrington;
      4. That the undersigned and their attorney should review a title insurance policy or title insurance commitment for the subject property to determine what covenants, conditions and restrictions of record are in fact applicable to the subject property; and
      5. That, in consideration for the issuance of a building permit, the undersigned further agree to hold harmless and indemnify the Village, its officers, employees and agents, from any cost, claim, expense or liability to the Village, resulting directly or indirectly from, or related to, any violation by the undersigned of such covenants, conditions and restrictions of record as may be applicable to the subject property, including reasonable attorneys' fees and other costs of defense.
   D.   Foundation Survey: An appropriate number of copies of such foundation survey, as determined by the Building Commissioner, shall be provided to the Building Commissioner for review and approval after completion of the foundation for any new building and/or any addition to an existing building and before any further work on the building or addition shall proceed, and in the event such foundation survey is not submitted to the Building Commissioner within fifteen (15) days after the foundation has been completed, the building permit in question shall be automatically suspended without further action by the Building Commissioner.
