(A) No permits shall be issued for construction, repair or installation upon any street, sidewalk, alley or other public way unless evidence of appropriate insurance has been provided. The applicant shall provide a certificate of insurance demonstrating evidence of workers compensation coverage; commercial general liability coverage; and automobile liability coverage and any other insurance that may be required due to the nature of the work to be performed. Minimum coverages shall be approved by the Director of Public Works dependent upon the scope of work to be undertaken.
(B) The coverage shall be provided by an institution with a financial rating of not less than XIII, and a holder rating of no less than “A” as assigned by the Best Key Rating Guide.
(C) The certificate of insurance shall also name the village as an additional insured under the policy coverage.
