§ 36.01 AMBULANCE SERVICE FEES.
   (A)   All persons utilizing emergency response and emergency ambulance services, as hereinafter defined, shall pay to the village a fee for emergency ambulance response, treatment and transport services, rendered within or outside of the corporate limits of La Grange Park. This fee shall be paid within 60 days of the mailing or delivery of an invoice therefor. The fee, per person, for the two levels of emergency ambulance services provided shall be:
      (1)   Basic life support services: $1,600;
      (2)   Advanced life support services: $1,600.
   (B)   In addition to other fees set forth in this section, a mileage fee shall be charged for emergency medical transportation. The mileage fee shall be calculated in fractional miles for transportation from the scene to the hospital. The per mile fee for emergency medical transportation to anyone transported, resident or non resident, rendered will be $25 per mile.
   (C)   All residents and non-residents shall pay to the village a fee for emergency ambulance response when advanced life support services (as defined below) are provided, but the patient refuses transportation. This fee shall be paid within 60 days of the mailing or delivery of an invoice thereof. The fee, per person, for advanced life support services without transport, shall be $500.
   (D)   The two levels of services for which fees are established herein are defined as follows:
      BASIC LIFE SUPPORT SERVICES. A basic level of pre-hospital emergency and non- emergency medical care generally including, but not limited to, airway maintenance, oxygen administration and ventilation (breathing) support, CPR, hemorrhage control, splinting of fractures, management of spinal injury, protection and transportation of the patient in accordance with accepted procedures.
      ADVANCED LIFE SUPPORT SERVICES. An advanced level of pre-hospital emergency and non-emergency medical care that includes all basic life support measures, plus invasive medical procedures including intravenous therapy, cardiac monitoring, pacing and defibrillation, administration of medication and solutions, use of ventilation devices, and other procedures governed by state law and performed under medical control.
   (E)   The Village Manager is hereby authorized to waive, in whole or in part, the fees set forth in this section on a case by case basis based upon a review of all the pertinent facts and circumstances, when the Village Manager finds that it is in the best interests of the village to do so.
   (F)   For the purposes of this section, RESIDENTS are defined as individuals who are domiciled within the Village of La Grange Park.
