§ 95.36 VIOLATION NOTICE AND REPORT.
   (A)   The Chief of Police shall serve a violation notice and report upon the owner or person in charge of the property on which a nuisance exists, or upon the person causing or maintaining the nuisance, requiring that the nuisance be abated within five calendar days from the service of the violation notice and report.
   (B)   The violation notice and report shall contain the following:
      (1)   The name and address of the owner or person in charge of the property on which a nuisance exists, or upon the person causing or maintaining the nuisance, served with the violation notice and report;
      (2)   A description of the type and nature of the nuisance;
      (3)   The date and time the nuisance was observed;
      (4)   A statement of acts necessary to abate the nuisance; and
      (5)   The penalty assessed pursuant to § 95.99.
