Applications for licenses hereunder shall be made upon such forms as may be prescribed by the City. Applicants shall file with the City Clerk an application in writing that shall give the following information:
A. The name and address of the applicant and the owner of the ambulance service and ambulance;
B. The trade or other name, if any, under which the applicant does business and proposes to do business;
C. The training and experience of the applicant in the transportation and care of patients;
D. The location and descriptions of the place or places from which the applicant intends to operate;
E. Description of system for providing medical direction;
F. Applicant shall submit with the application a Certificate of Insurance meeting the requirements outlined in 4-11-10
and provide an inspection report for each vehicle showing proof of annual ambulance vehicle inspection conducted by an Automotive Service Excellence (ASE) certified mechanic as outlined in section 4-11-8
.
G. An accompanying license fee in an amount as may be set from time to time by resolution of the City Council;
H. Evidence of its current ability to meet the eligibility criteria for an ambulance service license set forth in Section 4-11-6
of this chapter, along with a statement of its agreement to continuously meet such criteria during the period covered by the license;
J. Such other information as the City Council shall deem reasonably necessary to a fair determination of compliance with this chapter. (Ord. 23-09, 7-17-2023)
