§ 150.50  APPLICATION.
   (A)   The current owner of record or subdivider shall file an application for final plat approval of a major subdivision with the staff at least 30 calendar days before the desired date of the public hearing. The application shall be accompanied by a certified check or money order in the amount consistent with the current fee schedule. Written application on a form approved by the staff must be filed along with one Mylar original and 12 copies of the final plat. As per state statutes, the final plat may include all or only part of the approved preliminary plan.
   (B)   The original final plat shall be drawn at a commonly used scale on Mylar film with a sheet size of 18 inches by 22 inches. The following basic information shall be shown on the final plat:
      (1)   Location map showing:
         (a)   Subdivision name and location;
         (b)   Any thoroughfares related to the subdivision;
         (c)   Existing schools, parks and playgrounds serving the area to be subdivided, and other community facilities; and
         (d)   Title, scale and north arrow.
      (2)   Names and addresses of the current owner(s) of record and subdivider(s); and the consultant or surveyor who prepared the final plat;
      (3)   All streets, alleys and rights-of-way on and adjoining the site of the proposed subdivision; showing the names and including right-of-way widths, and any limitations on the public or private use;
      (4)   Accurate locations and dimensions of easements for utilities and other purposes, and any limitations on the semi-public or community use;
      (5)   Main streams, ditches and/or any other potentially affected waterway;
      (6)   Parcels of land to be dedicated or reserved for schools, parks, playgrounds or other public, semi-public or community use, with accurate dimensions;
      (7)   Layout of lots with accurate lot numbers;
      (8)   The location and accurate dimensions of all tract boundaries, lot lines, corners and monuments with references to appropriate land system boundaries or corners (i.e., donation, survey, section, township, range and the like);
      (9)   Accurate legal description of the subdivision boundary;
      (10)   All boundary survey information shall comply with the minimum standards for competent practice of land surveying as outlined in 865 I.A.C. 1-12;
      (11)   A reference to a previously recorded survey of the tract(s) containing the subdivision as required under 865 I.A.C. 1-12;
      (12)   An acreage breakdown of all contributing tracts and tax parcels into all new streets, tracts or lots, including tax parcel numbers;
      (13)   Substantiating curve data for all curves included in the boundary layout;
      (14)   Location, type of material and size of all monuments and lot markers;
      (15)   Setback lines and dimensions;
      (16)   Legend and notes;
      (17)   Scale, north arrow and date;
      (18)   Source of title of the owner to the land as shown by the last entry in county records;
      (19)   The protective covenants or private restrictions to be incorporated in the plat of the subdivision or a reference to their recordation in other public records;
      (20)   Certification of dedication of streets and other public property or reservations for private use;
      (21)   Name, date and signature of current owner(s) of record and subdivider(s);
      (22)   Name, date, seal and signature of registered land surveyor;
      (23)   Certificate for approval by the Area Plan Commission; and
      (24)   Certificate for approval by the applicable government entity.
