(a) All original applications for licenses required by this chapter shall be made to the City Clerk, in writing, upon forms to be furnished by the City Clerk and shall contain the following information:
(1) The applicant's present occupation, trade or business and place of business;
(2) If an individual or partnership, the applicant's place of residence and place of business for the five years next preceding the date of the application;
(3) If a corporation, the applicant's place of business for the five years next preceding the date of the application;
(4) The nature and location of the intended business or enterprise;
(5) The number of employees expected in the next year;
(6) A description of the merchandise to be sold, if for a vendor;
(7) The employer's tax identification number or social security number; and
(8) Such other information concerning the applicant and the applicant's business as may be reasonable and proper, having regard to the nature of the license desired.
(b) With each application, the applicant shall deposit a fee as stated in the Fee Schedule in § 236.07 of the City of Kirtland Codified Ordinances.
(c) No person shall knowingly make any false statement or representation in the license application.
