§ 114.25 RECORDS TO BE MAINTAINED ON ACTIVITIES.
   (A)   Each body piercing establishment shall maintain permanent records for each patron. Before the body piercing operation begins, the patron shall be required to personally complete a form to be provided by the body piercing establishment, containing such information as may be required by the establishment, including but not necessarily limited to: name, legal proof of age, address, and telephone number. In the event that the patron is less than 18 years of age, then the records required by this section shall also include the original, executed form required by § 114.02(A)(2)(f).
   (B)   Such records shall be maintained on the premises for a period of no less than two years and shall be available for examination by the town, health department, and/or the Department of Health and Human Services.
