§ 97.041 APPLICATION FOR PERMIT; FEE.
   (A)   Any person desiring to dig or excavate in the streets, alleys, parking strips or other property belonging to the city must first file an application with the City Engineer asking for a permit to dig and excavate, which application must set forth the following:
      (1)   The place where said digging or excavating is to be done.
      (2)   The purpose of said digging or excavating.
      (3)   The time the work is to start and the time same is to be completed.
      (4)   The person or persons doing the work.
      (5)   The party on whose behalf said work is being done.
      (6)   Any other information that the City Engineer shall require to be given in said application.
   (B)   It is further provided that the City Engineer shall approve all applications for permits to dig or excavate, before the issuance of a permit, which permit shall be for a period of 30 days and shall be issued by the Clerk on receipt of an approved application, and said applicant shall pay a fee of $1 for said permit, which fee shall accompany the application.
