§ 111.58 FINES.

   For the violation of any of the following provisions of this chapter, penalties shall be chargeable to the letter of credit as follows:

   (A)   For failure to complete system construction and provide service in accordance with § 111.31, unless the Cable Board specifically approves the delay by motion or resolution, due to the occurrence of conditions beyond the company's control, the company shall pay $500 per week for each week, or part thereof, the deficiency continues.

   (B)   For failure to provide data, documents, reports, or information as required by §§ 111.38, 111.40, 111.41, and 111.42, the company shall pay $50 per day for each day the violation occurs or continues.

   (C)   For failure to test, analyze and report on the performance of the system following a request pursuant to § 111.36, the company shall pay $50 per day for each day, or part thereof, that such noncompliance continues.

   (D)   For failure to comply with the operational standards following the Cable Board's resolution directing the company to make improvements pursuant to § 111.34, the company shall forfeit $200 per day or part thereof that the violation continues.
