§ 112.02 APPLICATION FOR PERMIT; CONTENTS.
   (A)   Any person desiring to secure a solicitor’s permit shall apply therefor in writing over his or her signature to the Chief of Police on forms provided by the village. The application shall state:
      (1) The name and home address of the applicant;
      (2)   The name and address of the person, company or organization by whom the applicant is employed or for whom the applicant is acting;
      (3)   The length of service of the applicant with that employer;
      (4)   The place of residence and nature of the employment of the applicant during the last preceding year;
      (5)   The nature or character of the goods, wares, merchandise or services to be offered by the applicant, or his or her employer;
      (6)   The purpose or purposes for which the payment or contribution of money or the pledge of financial support will be solicited or accepted if other than in exchange for the purchase of goods, wares, merchandise or services;
      (7)   The proposed dates on which the soliciting activity will take place within the village; and
      (8)   The personal description of the applicant.
   (B)   The application shall be accompanied by credentials and other evidence of the identity of the applicant and the employer or other organization for whom the applicant is acting as are reasonably required by the Chief of Police as necessary to verify the identity of the applicant and reasonably and accurately describe the nature of the applicant’s activity within the village.
