§ 111.088 CERTIFICATE OF INSURANCE.
   Upon request of the Village Manager, the applicant shall provide a certificate of insurance in the amount of $1,000,000, or such greater amount as the Village Manager deems necessary in light of the nature of the event, activities, persons, property and risks involved, as evidence of general liability coverage including bodily injury and property damage, and automobile liability, if applicable. Such certificate shall name the village and its corporate authorities, officers, officials, boards, commissions, employees, attorneys, agents and representatives as additional insureds with respect to any and all claims which arise out of, or are related to, the applicant’s activities related to the permit. Upon request of the Village Manager, the applicant will also provide proof that worker’s compensation and employer's liability insurance have been provided for any and all employees of the applicant.
