Applications for all licenses and permits required by the provisions of this code or any other ordinance of the village, shall be made in writing to the Village Manager, unless express provision to the contrary is made, on forms provided by the village, and shall state the name of the applicant, the permit or license desired, the location to be used if any, the time covered and the fee to be paid and any additional information as is required by this chapter or any other ordinance applicable.
