Application for the permit required under this subchapter shall be filed with the Village Manager, and shall state the location of the intended excavation or tunnel, the size thereof, the purpose thereof, the person doing the actual excavating and the name of the person for whom the work is to be done. The fee for the permit shall be for the amount established in the annual fee ordinance.
