§ 151.048 CERTIFICATE OF APPROVAL.
   (A)   The following certifications shall be placed on the final plat and are mandatory for the final approval and recording of the plat:
      (1)   Certification by a registered surveyor to the effect that the plat represents an accurate survey and that all monuments shown thereon actually exist and that their location is shown correctly, and by a registered engineer that all improvement plans are accurate;
      (2)   Notarized certification of ownership of the land being platted and acknowledgment and adoption of the plat and dedication of streets and open spaces;
      (3)   Certification of approval by the Village Planning Commission;
      (4)   Certification of approval by the County Board of Health where sewer or water is not available;
      (5)   Certification of approval by the Auditor of Erie County stating that all taxes and assessments on the land in the subdivision have been paid;
      (6)   Certification by the Village Engineer that the subdivider has complied with one of the following alternatives:
         (a)   All improvements have been installed in accordance with the requirements of this chapter and in accordance with any other recommendations of the Village Engineer, the Erie County General Health District, the Village Council, and the Village Planning Commission; or
         (b)   A financial guarantee has been posted in the name of the village with the proper agency in sufficient amount to insure completion of all required improvements;
      (7)   Certificate of approval by the County Sanitary Engineer;
      (8)   A statement shall be placed on the plat and all deeds requiring the future connection to central water or sewage facilities when either or both become available;
      (9)   Certificate for acceptance by the Village Council of any street whose dedication has been requested;
      (10)   Such other certificates, affidavits, endorsements, or dedications as may be required by the Village Planning Commission in the administration of the regulations;
      (11)   Certification of approval by the Village Council and the ordinance number authorizing acceptance; and
      (12)   Certificate of County Recorder.
   (B)   The checklist attached as Appendix § 5 will be used by the Planning Commission to assure that final plat requirements are met and can be used as a guide to assist the subdivider in meeting the final plat information requirements.
