§ 110.02 APPLICATION FOR LICENSE.
   (A)   All original applications for licenses, unless otherwise specifically provided, shall be made to the Village Clerk or other duly authorized issuing authority, in writing upon forms to be furnished by him and shall contain:
      (1)   The name of the applicant and of each officer, partner or business associate who has held an ownership interest in the business for the last three years;
      (2)   His present occupation and place of business (this information shall be included for each officer, partner or business associate who has held an interest in the business for the last three years);
      (3)   His place of residence for five years next preceding the date of application (this information shall be included for each officer, partner or business associate who has held an interest in the business for the last three years);
      (4)   The nature and location of the intended business or enterprise;
      (5)   The period of time for which the license is desired;
      (6)   A description of the merchandise to be sold, if for a vendor;
      (7)   Such other information concerning the applicant and his business as may be reasonable and proper, having regard to the nature of the license desired, i.e., vendor's license number if any.
   (B)   Renewal of an annual license may be granted to a licensee in good standing upon the original application, unless otherwise provided.
   (C)   With each original or renewal application, the applicant shall deposit the fee required for the license requested. The fee for the original license shall be the sum of $125, and the fee for each annual renewal license shall be $75.
   (D)   In the event that there is any change or transfer of the ownership of a business or enterprise, a new application shall be required within 60 days of the change or transfer of ownership. Further, the new owner shall deposit the fee required for an original license with the application reflecting said change or transfer of ownership.
   (E)   Said license shall be posted in a conspicuous location visible to the general public on the premises of the business or enterprise.
   (F)   It shall be unlawful knowingly to make any false statement or representation in the license application.
