A temporary food service establishment is any food service establishment that operates at a fixed location for a period of time not to exceed fourteen (14) consecutive days in conjunction with a single special event or celebration.
(a) Application Required:
1. All operators of a temporary food service establishment shall complete and submit an application as prescribed from time to time by the health officer for such activities, to the village of Justice building department. All information as requested in the application shall be provided, including but not limited to, the date of the event,
location dates and times of operation, vendor’s name, business address, phone, vendor’s home address, number and expiration date of food handlers certification and copies of food handlers certification, foodstuffs to be served, names and address of all approved sources of foodstuffs, equipment that will be used to maintain hot or cold temperatures during the event, and the operators and organizer’s signature.
2. Approval shall be by the health officer.
(b) Fees:
1. A health inspection fee, as set forth in section 13-1 of this code, shall be charged for each day that the temporary food service establishment is in operation.
2. The health inspection fee shall be paid by the organizer/event sponsor or the temporary food service establishment operator as determined prior to the event and submitted with the application. Any application received without the health inspection fee shall be denied.
(c) Application Deadline: All completed applications, with the appropriate copies of licenses and fees attached, shall be filed in the building department, no later than fourteen (14) days prior to the event. Late entries shall be handled on a case by case basis by the health officer whose decision shall be final.
