(b) The following information shall be provided the municipal services administrator upon such registration, as a condition of doing business within the village:
(1) The name of the owner, owners, or principal shareholders;
(2) The addresses of offices located within the corporate boundaries of the village;
(3) A brief description of the nature of the business conducted;
(4) Such other information as may be required by the board for the protection of the public health, safety or welfare of the patrons of such business, their employees and the general public;
(5) Home occupation licenses in multiple-family or rental units must have a letter of approval from the landlord and/or manager or have proof of ownership.
(c) The provisions of section 3-7 of this chapter shall apply to such registration.
