(A) Upon exhaustion of all accrued paid leave time, any request for unpaid leave will be submitted to the employee’s supervisor and the Mayor. The Mayor and supervisor will decide whether to grant the unpaid leave request and other terms of the leave depending upon the reasons for the requested leave and the needs of the department and the city.
(B) If granted an unpaid leave of absence, an employee will not be compensated and will not receive any other employee benefits. As such, the employee must make an election as to state continuation coverage for group health care for the employee as well as any qualified dependents. In addition, the employee will be subject to the terms and conditions of the specific plan to continue any optional benefits upon prepayment by the employee of the applicable contribution or premium during the period of the absence. Any failure by the employee to prepay any optional benefits may result in termination of the benefit.
(C) Employees on unpaid leave will not accrue any vacation or sick leave time during the unpaid absence.
