Application for such license shall be made in writing upon forms furnished by the City Clerk and shall be filed with the Clerk. Such applications shall give the full name of the applicant, his residence and the place where he intends to do business. If said applicant is a partnership, the names of all partners and their street addresses shall be given; and if a corporation, the names and addresses of all officers shall be given.
