(A) Any person desiring to conduct a convalescent home or home for the aged in the city shall make an application in writing to the Department of Health, in triplicate, on a form which shall conform to the general provisions of this chapter. One copy of the application shall be forwarded at once to the Fire Department, and one copy to the Building Inspector; and the report of their recommendations shall be made to the Department of Health. The applicant shall also submit satisfactory evidence that he is of reputable and responsible character, and in the event the applicant is an association or corporation, like evidence shall be submitted as to the members thereof and the person in charge. Applicants shall, in addition, furnish satisfactory evidence of their ability to comply with the minimum standards of this subchapter and all the rules and regulations adopted thereunder.

(B) Each application to the City Council for a license to conduct a convalescent home or home for the aged shall be accompanied by a fee to be determined by the number of beds available and approved for patients thereof. Those with ten patients or less shall pay a fee of $50; those with over ten patients shall pay a fee of $100 annually.
