§ 113.092  LICENSE APPLICATION; CONTENTS.
   (A)   The application for such license shall contain the following information:
      (1)   Name and address of the applicant, age, date and place of birth;
      (2)   Prior conviction of applicant, if any;
      (3)   Place where jukebox machine is to be displayed or operated and the business conducted at that place;
      (4)   Description of jukebox machine to be covered by the license, mechanical features, name of manufacturer, and serial number.
   (B)   No license shall be issued to any applicant unless he shall be over 21 years of age and a citizen of the United States.
   (C)   Failure to obtain a license in accordance with the provisions of this subchapter shall be deemed a violation of this subchapter on the part of the owner of the jukebox machine and/or the person or persons, firm, corporation or association on whose premises any such jukebox machine is placed and punishable in the manner set forth in this chapter for violations of this subchapter herein.
